Make inside ear.

Select faces and extrude.

Pull vertexes.

Select face and extrude.

Select edges ear border and extrude. Make shape for back of ear.

Place ear on the head. Rotate ear a little.

Select edges border on head. And extrude it. Snap head vertexes to ear vertexes.
